Ethnic Rhinoplasty Revision Case Study #1

The above patient is a 33 year old African-American female who had 2 previous Rhinoplasty procedures with another surgeon in 2006 and 2008. In 2006, her surgeon placed a Gore-tex implant with the intent of augmenting her low nasal bridge. Instead, the improper placement of the implant resulted in a Pollybeak Deformity. This is illustrated in the photo below. The same surgeon performed her second African-American Revision Rhinoplasty in 2008. That procedure yielded no discernible results and left the patient very dissatisfied. Upon consulting with Dr. Slupchynskyj, she complained of her down-drooping, humped nose and nasal width. In 2010, she decided to have her final African-American Revision Rhinoplasty with Dr. Oleh Slupchynskyj.

When a facial plastic surgeon chooses to insert a nasal implant during a Rhinoplasty procedure, he or she must consider several factors, which include implant size, implant material and the insertion location. Upon examining this patient, Dr. Slupchynskyj could clearly see that her Gore-tex implant was placed improperly by the previous surgeon and was residing in the wrong tissue plane. In addition, the implant was too large for her nose. This left the patient with a Pollybeak Deformity: a condition in which over-projection of the nasal bridge results in a downturned nasal tip that sometimes resembles a parrot's beak.

In order to correct this Pollybeak Deformity created by her previous Rhinoplasty and provide the appropriate bridge elevation the patient desired, Dr. Slupchynskyj used an Open Rhinoplasty approach, removed her Gore-Tex graft and replaced it with a custom carved SLUPimplant™.
